What Is Medicare?

Medicare is the federal health insurance program for people 65 and older, as well as certain younger individuals with disabilities or specific medical conditions. It was established in 1965 and today covers more than 65 million Americans. If you live in Coral Springs, West Palm Beach, Boca Raton, or anywhere in South Florida, understanding your Medicare options is one of the most important financial decisions you will make as you approach retirement.

Medicare is divided into several parts. Part A covers hospital stays, skilled nursing facility care, hospice, and some home health services. Part B covers doctor visits, outpatient care, preventive services, and durable medical equipment. Part C, also known as Medicare Advantage, bundles Parts A and B (and usually Part D) into a single plan offered by private insurers. Part D provides prescription drug coverage.

Original Medicare (Parts A and B) covers roughly 80% of approved medical costs, leaving you responsible for deductibles, copayments, and coinsurance. That is where Medicare Supplement plans (Medigap) or Medicare Advantage plans come in. These are the two primary paths people take to fill the coverage gaps, and each one works quite differently.

Who Needs Medicare Coverage?

If you are turning 65, you are eligible for Medicare regardless of your health status. Your Initial Enrollment Period (IEP) starts three months before your 65th birthday and extends three months after it, giving you a seven-month window to enroll without penalties. Missing this window can result in late enrollment penalties that last for the rest of your life.

Medicare is also available to people under 65 who have received Social Security Disability Insurance (SSDI) for 24 months, or who have been diagnosed with End-Stage Renal Disease (ESRD) or Amyotrophic Lateral Sclerosis (ALS). If you are still working at 65 and covered by an employer plan with 20 or more employees, you may be able to delay enrollment without penalty.

Broward and Palm Beach counties alone account for hundreds of thousands of enrollees, and the number of available plans in our region can be overwhelming. How Medicare Enrollment Works

There are several enrollment periods you need to be aware of, and each one determines what changes you can make to your coverage.

The Initial Enrollment Period (IEP) is your first opportunity to sign up for Medicare. It is tied to your 65th birthday and lasts seven months. During this period, you can enroll in Parts A and B, choose a Medicare Advantage plan, add a Part D prescription drug plan, or purchase a Medigap supplement.

The Annual Enrollment Period (AEP) runs from October 15 through December 7 every year. This is when you can switch between Original Medicare and Medicare Advantage, change your Advantage plan, or switch Part D plans. Any changes you make during AEP take effect on January 1.

Special Enrollment Periods (SEPs) are available when you experience qualifying life events such as moving to a new service area, losing employer coverage, or qualifying for Medicaid. These give you a window outside of AEP to adjust your coverage.

Medicare Supplement (Medigap) plans work alongside Original Medicare. You pay your Part B premium plus a Medigap premium, and the supplement covers most or all of the out-of-pocket costs that Medicare does not pay. The major advantage is freedom: you can see any doctor or specialist in the country who accepts Medicare, with no referrals and no network restrictions. Plans are standardized by letter (Plan G, Plan N, etc.), so the benefits are the same regardless of the insurance company. The difference between carriers is premium cost and customer service.

Medicare Advantage (Part C) plans replace Original Medicare. They are offered by private insurance companies like Humana and Cigna and must cover everything Original Medicare covers, but they often include additional benefits like dental, vision, hearing, fitness memberships, and even over-the-counter drug allowances. The trade-off is that Advantage plans use provider networks (HMO or PPO), and you may need referrals to see specialists. Monthly premiums are often lower, sometimes even $0, but out-of-pocket costs at the point of care can be higher.

In South Florida, Medicare Advantage plans are especially popular because the competitive market means insurers offer rich benefits to attract enrollees. However, that does not mean Advantage is automatically the best choice. If you travel frequently, see out-of-network specialists, or have significant health conditions, a Medigap plan with its nationwide coverage may serve you better.

That creates a highly competitive marketplace, which is good news for consumers because insurers compete aggressively on benefits and pricing. However, it also means there are dozens of plans to sort through in most zip codes, making expert guidance essential.

South Florida is home to some of the nation's top hospitals and healthcare systems, including Cleveland Clinic Florida, Baptist Health, and HCA Florida. Florida does not have a Medigap birthday rule like some states, so pricing for Medicare Supplement plans is based on attained age in most cases.
